1 sourceThe US Food and Drug Administration approved Pfizer's PARP inhibitor talazoparib (Talzenna), taken with enzalutamide, on 26 October 2026 for adults with metastatic prostate cancer that is naive or sensitive to androgen pathway modulation and carries homologous recombination repair (HRR) gene mutations 1. Patients must be identified by an FDA-authorised test, and the agency had already cleared the FoundationOne CDx and FoundationOne Liquid CDx assays as companion diagnostics for this pairing 1.
What the trial found
The decision rests on TALAPRO-3, a randomised, double-blind, placebo-controlled study of 599 patients assigned 1:1 to daily enzalutamide with either talazoparib or placebo, all on continuous androgen deprivation therapy 1. HRR status was established prospectively by next generation sequencing of tumour tissue or circulating tumour DNA, across 12 genes including BRCA1, BRCA2, ATM, CDK12 and PALB2 1.
The primary measure was radiographic progression-free survival assessed by the investigator. Median rPFS had not been reached in the talazoparib arm, against 45.8 months with placebo plus enzalutamide, a hazard ratio of 0.48 (95% CI 0.36 to 0.65; p<0.0001) 1. Overall survival was a secondary measure and the data were immature when the rPFS analysis was run 1.
The label carries warnings for myelodysplastic syndrome and acute myeloid leukaemia, myelosuppression and embryo-fetal toxicity 1. The recommended talazoparib dose is 0.5 mg by mouth once a day alongside enzalutamide, continued until progression or unacceptable toxicity, and patients also need a GnRH analogue or a prior bilateral orchiectomy 1.
What it means for the market
The approval moves a PARP inhibitor combination into a hormone-naive or hormone-sensitive metastatic population, which ties prescribing to genomic testing at an earlier point in the disease. Because eligibility depends on an FDA-authorised test, the decision also brings demand for HRR profiling, by tissue or by liquid biopsy, forward in the patient pathway, which matters to diagnostic suppliers and to the oncology services that order the tests 1.
The review ran under Project Orbis, the FDA Oncology Center of Excellence framework for concurrent submission and review of cancer drugs with partner regulators. The FDA worked with Australia's Therapeutic Goods Administration and Health Canada, and their reviews are still ongoing 1. The application was granted priority review, and Pfizer used the Assessment Aid, a voluntary submission format meant to speed the FDA's assessment 1. For regulatory teams, the case is another example of a large oncology filing moving through the US agency on an expedited, multi-country track.
Caveats
The efficacy finding is for radiographic progression on investigator assessment, and survival data were not mature at the time of analysis 1. The FDA notice covers the US decision only. Full prescribing information is due to be posted on Drugs@FDA, and the Australian and Canadian reviews have not yet concluded 1.
